Comprehensive Record System

UMED's Comprehensive Record System (CRS) is a physician-completed medical record service. 

Cost: Only $200 per year.

Here's how UMED's Comprehensive Record System works:

UMED sends you the easy to complete Comprehensive Record System Questionnaire # 1.  You provide us some basic information including the names of the doctor(s) that you would like us to contact along with the annual subscription fee.

• UMED sends the detailed Comprehensive Record System Questionnaire # 2 to your physician(s), along with a fee to cover their costs, and we follow up to make sure they complete it.
• When we receive your completed CRS questionnaire with your payment, our nurses edit your record for inconsistencies and then add your medical history to our database
• UMED immediately sends you a copy of your completed CRS record, a UMED ID card and other identification, and instructions on how to keep your record up-to-date.
• You can update your record as often as you like at no charge.  (There is a fee, however, every time we need to contact your physician(s) to obtain an update.)
• In an emergency we will fax your medical records to your attending emergency physician thus ensuring rapid, effective treatment.
• For day-to-day healthcare management, UMED can forward your medical records to a specialist or new physician.  This speeds admissions and prevents annoying, repetitive paperwork.

UMED's Comprehensive Record System can give you the peace of mind of knowing that you have put your medical records where they can save your life.  In addition, CRS will allow you to take control of your medical records thus reducing duplicate paperwork and admission delays.
